Where does individual provider responsibility and accountability begin and end in a culture in which medical errors are recognized as being a failure of the system? True reflection requires analysis and higher level thinking, as well as self-awareness and risk taking. Genuinely reflective writing typically involves “revealing” anxieties, errors and weaknesses, as well as strengths and successes.
Although the focus of this reflective discussion is self-analysis, cited references should be included to support your analysis. See the rubric for reference expectations.
