I need a response to this discussion: A Community Health Assessment Model refers to a state, tribal, local, or territorial health assessment that identifies key health needs and issues through systematic, comprehensive data collection and analysis (What, 2018). This assessment model allows for a population to indicate problems in their community, strategies to solve those problems, and tools and processes to achieve the desired outcomes (What, 2018). The Community Health Assessment Model that I chose to discuss is the Mobilizing for Action through Planning and Partnership (MAPP). The CDC describes this model as the framework for community health improvement planning at the local level that has a strong emphasis on community engagement and collaboration (Common, 2015). The benefits of using this specific model includes implementing long-term strategies that address problems and having the community take ownership of the process to increase the credibility and sustainability of health improvement efforts (Mobilizing, 2021). This model includes four phases; organize for success and partnership development, visioning, the four assessments, identify strategic issues, formulate goals and strategies (Mobilizing, 2021). These phases can take a while to develop due to the increased community involvement, ongoing assessments of issues, and new developments of short-term goals. A public nurse may come into this model during phase one and two, where a steering committee is formed and the local public health system is involved. In this phase the public nurse is able to give the process legitimacy by offering strong initial support and providing the range of expertise necessary to develop the substance of the plan (Mobilizing, 2021). During phase three, the public nurse has the responsibility to answer questions regarding the local public health system and their components, activates, competencies, and capacities along with what kind of essential services are provided to the community (Mobilizing, 2021).
