References must be up to date from 2013-2021 Assignment 3: Professional care plan Assignment overview The care of the older person is complex and requires inter-professional involvement and culturally safe practice. This complex care often relates to managing multiple health conditions, pharmacological requirements, social vulnerability and physical and psychosocial issues. These needs typically interact with and exacerbate one another leading to individuals experiencing several problems simultaneously. Based on the case you assessed in Assignment 2: Case study, you will develop an interdisciplinary care plan that addresses the needs of your case and the problems you have identified with their underlying pathophysiology. You will summarise and justify this care plan in the form of a written essay, with the care plan as an appendix. This assignment supports unit learning outcomes 2 and 3. Assignment details Based on the problems and underlying pathophysiology you identified via the completed health assessment in Assignment 2, you must now prepare a professional plan for your case. The care plan must explore the roles of at least three different health professionals and consider culturally safe practices relevant to the case. You will then use this professional plan to write your essay. This completed template should be included with your submission as an appendix that will contribute to your word count. Assignment 3 structure Introduction (100 words) Provide a brief introduction of what you intend to discuss in the essay. Body (1900 words) This section should provide a written summary and justification of your professional plan using supporting evidence from the literature. You’ll need to outline the key components of your professional plan. Be sure to include: A summary of the identified problems and justification of these linked to the underlying pathophysiology. A summary of short and long-term goals (recommend no more than five goals) to address the identified problems. o Goals need to be realistic, achievable and person-orientated, and clearly linked to your case study. Identification and critical examination of specific strategies or interventions to support your case study from the perspective of at least three health professionals. Identification of the tasks/actions necessary to complete each strategy/intervention, as well as who will perform these tasks. o You must reference the challenges discussed in Module 6: Pharmacology and deprescribing and Module 7: The standards and challenges of care planning in older people. Evaluation of the effectiveness of the plan (. how the achievement of goals will be evaluated).
